Thereby, the counseling sessions conducted by the Social Service Department, often times enables patients to better cope with their daily activities by providing much needed motivation and hope.
Follow-ups
To ensure that SIRC provides an effective service, monitoring the patient after he/she returns home is essential. Therefore, the Social Service Department corresponds with discharged patients and coordinates home visits. In cases where a home visit is not possible, the Department follows up with the patient via telephone or mail. The follow-up services helps SIRC assess the discharged patient's psychological, physical and economic status in their home environment. If the patient is facing any challenges reintegrating, SIRC attempts to assist the patients with their predicaments.
